To make a complaint, please ensure that you read the Fact Sheet and Complaints Policy documents before submitting your complaint.
Important Note: A complaint cannot be used to change a decision of the Tribunal or to change the Tribunal Member hearing a matter.
Then, complete the MHRT Complaints Form (PDF) and send to MHRT-MHRT@health.nsw.gov.au.
Please note that any complaint of breach of the Charter of Victims Rights is to be made to the Commissioner for Victims Rights/Victims of Crime Commissioner as set out in the Complaints Facts Sheet and Complaints Policy.
